Reservation Application & Contract Agreement
 
To confirm a reservation with the Greater Toledo Inline Hockey League, this form
and all accompanying paperwork must be completed, signed, and submitted along
with the reservation payment fee.

Reservations must be made and paid in full one
week prior to requested date.

Any players participating in the reservation who
would also like to participate in drop in before or
after the scheduled reservation must pay for drop in.

On rink officials are not provided, if you would like
to have on rink officials you must find your own.

Person making reservation request must be at least
18 years of age.
