ATTENTION PARENTS / PLAYERS:

This registration will allow you to provide your child's player information, your contact information and sign consents. Please read through each page carefully to ensure that all information is provided accurately. 

• Every parent / player is required to complete their own registration to acknowledge the code of conduct and liability waiver. If the player is a minor then a parent or guardian must register the minor player. Any registration by a 3rd party will not be accepted and it will be deleted.
